Payments

Brain and Behavior physicians do not accept or contract with any government insurance, private insurance, and/or 3rd party payers. This means that we are considered “out-of-network” providers. Individuals pay fee for service (FFS), with a balance due at the time of service. Professional Fees are generally charged on the same day of an appointment. A receipt for payment is available upon request.

Your insurance policy may have out of network benefits ranging from full reimbursement to no reimbursement. Upon request, an itemized form or "superbill" with an invoice of the services and associated fees may be provided to you. Depending on your insurance policy's out-of-network benefits, you may be able to submit this superbill and a claim form provided by your insurance for payment.

You are encouraged to call your insurance company and ask about what your insurance policy's out of network benefits are and the process to submitting a claim to your insurance company for payment to you directly. In some cases, there is a yearly deductible that needs to be met before you will receive payment. In other cases, you will also need to get an authorization to see an out of network provider before filing claims for payment.

Patients should be aware of the following:

1) Brain and Behavior psychiatrists DO NOT participate in Medicare and/or Medicaid. Due to pre-existing contracts, Brain and Behavior CANNOT provide services to Medicaid patients, Medicare patients, or any Medicare eligible patients (including those who are eligible for Medicare but use private insurance and/or use Medicare as a secondary insurance).

2) Brain and Behavior psychiatrists are not "in-network" with any commercial insurance and do not contract with any third party payers.

3) Brain and Behavior psychiatrists do not enter single case agreements with insurance companies.

4) Brain and Behavior psychiatrists are not responsible for filing with insurance or ensuring reimbursements from insurance plans.

5) Brain and Behavior requires an active credit card on file at all times. The active on file credit card is required in order to schedule all appointments and will be used for all professional fees and charges you will be billed for.
